Changes to Medication Policy
Under new guidelines set by the Pennsylvania Department of Health, the Carlynton School Board adopted a new medication policy that will change the way all medications are administered to students. The changes effect both prescription and over-the-counter drugs.
As of November 7, 2017, all medications for students MUST be brought to the child's school and picked up by a parent, guardian, or responsible adult, unless a licensed doctor writes an order allowing the child to carry the medication on their person (such as an inhaler).
No prescription or over-the-counter medication will be given to a student without a doctor's order AND the parent/guardian signature.
Parents are to use THIS FORM for authorization. It MUST be completed and signed by the parent/guardian and a Licensed Prescriber and is necessary for either/or prescribed medication or over-the-counter medication, such as Tylenol, Benadryl or Sudafed. This authorization form can also be found under the PARENT RESOURCES tab of the district website.
