For a smooth enrollment process, please print and complete the items listed below.  Along with these items, you will also need to provide the students
birth certificate, up to date immunizations and 2 proofs of residency within the district

* Please make note that the ACT 26 Certification only needs notarized for students in grades 4-12

*Please call to set up an appointment to Register your child(ren)


    If you have a child(ren) who will be leaving the district, please be sure to withdraw them at the building they were attending.  This allows a smooth transition for the new district they will be enrolling into.  There is only one simple form to fill out and return to the correct building.  Please see the attached form to complete at your convenience.  Thank you for your cooperation.
